OMH Foundation My Champion Program
Did a caregiver at Otsego Memorial Hospital (OMH) make a memorable difference for you or your family? Did they champion your healthcare experience?
If so, you can honor that special person in a highly visible way through the OMH Foundation My Champion Program. When you make a donation in honor of your OMH physician, nurse, caregiver, or volunteer, your OMH Champion will receive an acknowledgement letter and an OMH Champion pin to wear proudly on their lapel. It's more than a pin. When you take advantage of our My Champion Program, you also help ensure the future of outstanding healthcare here at OMH.

To honor your physician, nurse, or other caregiver, simply download the form below, write his or her name in the space provided, and mail it with your tax-deductible gift, payable to the Otsego Memorial Hospital Foundation. We will present the custom-crafted My Champion pin to your awardee, along with a note of thanks in your name.
